Ascend Home Health & Hospice is a locally owned, nurse-led provider of in-home health, hospice, and palliative care in Logan, Cache Valley, and across Northern Utah. Our skilled nursing team delivers post-surgery rehab, chronic disease management, wound care, physical therapy, and occupational therapy in the comfort of home. We care for seniors and adults managing dementia, Alzheimer's, CHF, COPD, stroke recovery, cancer, and end-of-life needs.
